Our Medical Social Work Services
Mercy Health has Medical Social Work staff available 16 hours a day, 7 days a week to assist you in making the best care decisions. Whether you're weighing the options for your own treatment or for a loved one who's unable to care for him or herself, we can help you understand insurance policies, search for proper living situations, help you obtain medical equipment and refer you to community agencies for ongoing support.
We offer assistance with mental health and/or substance abuse crisis. We can make referrals for inpatient or outpatient treatment, and direct you and your family to local support groups.
Services to Our Primary Care Network
We are able to work with your primary care doctor to help you coordinate services for you or a family member, including home medical care, locating an alternative living environment and preparing for discharge — even before you are admitted.
An acute rehabilitation social worker helps to make the transition from hospital to home as comfortable as possible. If you or a family member believes you aren't ready to go home, the social worker then looks for a place for you to receive further care.
Inpatient Social Work
In collaboration with other team members, the inpatient social worker will assist in planning for your discharge. The social worker will also find various community agencies and resources to assist in your post hospital recovery.
