Welcome to the Memory Clinic

The Memory Clinic at Mercy Health Partners provides high quality, compassionate care to older adults who are concerned about changes in their memory. Here, we specialize in the diagnosis and treatment of memory related conditions such Alzheimer's disease, mild cognitive impairment, and other various forms of dementia.

Our multidisciplinary team, consisting of a neurologist, psychologist, nurse, and social worker, will perform a comprehensive assessment and develop an individualized treatment plan that addresses your specific needs — and those of your family or caregiver, too. The comprehensive assessment will be conducted over the course of three appointments:

• Our nurse, psychologist and social worker will conduct a medical and clinical interview with you. Your family or companion will also meet with our nurse and social worker.
• You will undergo neuropsychological testing with the psychologist to determine your strengths and challenges in cognitive functioning.
• Finally, the neurologist will review your test results and case management data with the team to determine the diagnosis. You and your family will then consult with the entire Memory Clinic team.

The Memory Clinic team will work with you and your family to provide educational and support opportunities, as well as identify additional community resources.

How do I schedule an appointment?

Your primary care physician will need to call Centralized Scheduling at 231-672-4800. They will set up your appointments and medical tests: CT scan of the brain, EEG and labs. The medical tests will only be ordered if you have not had them done in the last six months.

Will insurance cover the cost?

Medicare and secondary insurance cover all or most of the fees for the evaluation. If there are concerns, please call customer service at 888-777-6433.
